Registration Form - Due Friday, October 3, 2008

All registration papers must be completed in FULL and turned into Mrs. Z (science teacher) by October 3, 2008. Placement on team will be on a first come first serve basis based on complete forms - including parent/guardian volunteer sign-up. If you need a form, you may pick one up from Mrs. Z (science teacher).
